Understanding BMI: What It Tells You (And What It Doesn’t)

It is the most widely used tool globally to categorize human weight. But in recent years, BMI has come under fire from fitness professionals and doctors alike for being an overly simplistic, and sometimes misleading, metric.
So, is BMI a useful tool for tracking your health, or an outdated mathematical formula? The truth lies somewhere in the middle.
What Exactly is BMI?
BMI is a simple mathematical ratio based on your height and weight. It was invented in the 1830s by Adolphe Quetelet, a Belgian mathematician and sociologist — notably, not a physician. Quetelet was trying to define the mathematical characteristics of the “average man,” not to develop a measure of individual health.
The formula is straightforward:
BMI = Weight (kg) / Height (m)²
This ratio yields a number, typically between 15 and 40, which places you into one of the following World Health Organization (WHO) categories:
- Underweight: Below 18.5
- Normal Weight: 18.5 to 24.9
- Overweight: 25.0 to 29.9
- Obese: 30.0 and above
Why Doctors and Insurers Love BMI
Despite its origins, BMI was adopted by the medical community and insurance industry in the 20th century because it is incredibly cheap, fast, and non-invasive. You don’t need expensive equipment or blood tests to calculate it — just a scale and a tape measure.
At a population level, BMI is actually quite useful. Studies tracking millions of people show a clear correlation between high BMI (the obese categories) and increased risk for conditions like type 2 diabetes, cardiovascular disease, hypertension, and certain cancers.
As a broad screening tool to identify people who might be at risk and require further testing, BMI serves a purpose.
The Flaws of the Formula
The problems with BMI arise when it is applied rigidly to individuals without context. The formula has several significant blind spots:
1. It Doesn’t Differentiate Between Muscle and Fat
This is the most famous flaw of BMI. Muscle tissue is much denser than fat tissue. A heavily muscled athlete, like a rugby player or a bodybuilder, will likely weigh enough to be categorized as “overweight” or even “obese” according to their BMI, despite having very low body fat and excellent cardiovascular health.
2. It Ignores Fat Distribution
Where you carry your fat matters as much as how much fat you have. “Visceral fat” — the fat stored deep in the abdomen around the organs — is far more dangerous to metabolic health than “subcutaneous fat” stored under the skin on the hips or thighs. BMI cannot tell the difference between the two.
3. It Doesn’t Account for Age or Gender
Women naturally carry a higher percentage of essential body fat than men. Older adults naturally lose muscle mass and gain fat as they age. A healthy 25-year-old man and a healthy 65-year-old woman might have the exact same BMI, but their body compositions will be radically different.
4. It Is Based on European Populations
Quetelet derived his formula exclusively from white European populations. Modern research shows that different ethnicities face metabolic risks at different body weights. For example, individuals of South Asian descent tend to develop diabetes and heart disease at lower body weights than populations of European descent. In India, many health organizations recommend lower cutoff points (e.g., an “overweight” cutoff of 23.0 instead of 25.0).
Better Metrics to Track Alongside BMI
If you are trying to get a clear picture of your health, you shouldn’t rely on BMI alone. You need to combine it with metrics that measure body composition.
- Body Fat Percentage: This tells you exactly what proportion of your weight is fat versus lean mass (muscle, bone, water).
- Waist Circumference: A simple tape measure around your belly button is an excellent indicator of dangerous visceral fat.
- Ideal Weight Formulas: Clinical formulas that provide a targeted healthy weight range based on height and gender.
